Nevada Medicaid’s PCS Program Offers
Distinct Choices for In-Home Care
Available In-Home Services
Nevada Medicaid’s Personal Care Services Program provides in-home
services to Nevada Medicaid Recipients (Consumers) who need
assistance to continue to live in their homes due to a qualifying disability
or health condition. Personal Care Aides (PCAs) are individuals who
meet the requirements specified in MSM Ch. 3500 whether they are
providing care to an individual who has chosen a Provider Agency or
Self-Directed Care model.
Approved in-home services may include assistance with Activities of
Daily Living (ADLs) such as:
• Bathing • Dressing • Grooming • Toileting
• Continence care Transfers • Mobility • Eating
In some situations, services may also include assistance with
Instrumental Activities of Daily Living (IADLs) such as:
• Meal Preparation Light housekeeping
• Laundry • Necessity shopping
Program Options
Nevada Medicaid Consumers have a choice between two distinct
programs, Provider Agency Care and Self-Directed Care through an
Intermediary Service Organization (ISO). Consumers should become
familiar with both options and choose the program that best meets their individual needs.
Provider Agency Care
Provider Agency Care is a traditional in-home care program available in areas where Provider Agencies are located
or provide service. This option is designed for Consumers who do not have the desire or ability to arrange and
direct their own care and services. The Consumer or their Personal Care Representative (PCR) delegates the
primary decision-making authority and responsibility of their services and care to a Provider Agency. The PCAs are
hired, trained, assigned, and supervised by the Provider Agency.
The Provider Agency is responsible for scheduling service delivery and for providing back-up caregivers as needed.
The Provider Agency maintains the employee/employer relationship with PCAs, schedules their assignments, and
administrates the paperwork and fiscal services.
Self-Directed Care/ISO
Self- Directed Care is available statewide in Nevada and is designed for Consumers who desire more independence
and control over their personal care services and caregivers. Under this option, the Consumer or their PCR have the
primary responsibility and authority to hire, train, schedule, supervise, and dismiss their caregivers if necessary.
The Consumer maintains control over how and when care is provided according to the approved tasks and hours on
the care plan.
With guidance and support from an Intermediary Service Organization (ISO), which provides fiscal and
supportive services, the Consumer or their PCR develops a care delivery and back-up plan, then arranges
and directs their own care and services.

Nevada Medicaid Personal Care
Services Program
Nevada Medicaid’s Personal Care Services Program is designed to assist, support, and maintain recipients
(Consumers) living independently in their homes and community. This program helps Consumers who need non-
medical caregiver assistance to remain in the comfort and security of their own homes rather than be placed in an
institutional setting.
Consumer Requirements:
Eligible for full Nevada Medicaid
Health condition or Disability
Require assistance with activities of daily living
Program Benefits:
Provides eligible individuals with in-home, personal care
assistance
Presents alternatives to facility-based care and
reduces need for emergency room, acute care

• The Consumer/PCR selects an Agency to

• The Agency is responsible for recruiting,
interviewing, hiring, training, supervising,
scheduling, and dismissing caregivers
• The Consumer/PCR is responsible for recruiting,

scheduling caregivers and directing his/her
personal care services.
• The Agency has full control over selecting
and scheduling caregivers to provide personal
care services.
• The Consumer/PCR has full control over
selecting and scheduling caregivers. Trusted
individuals of their choosing (except legally
responsible adults) can then be trained and
compensated to provide care.
• The Agency provides fiscal and personnel
services.
ISO provides fiscal and supportive services.
• Frequent supervisory visits are performed by
Agency to ensure program compliance.
• Initial visits are made by the ISO for enrollment.
Support is then provided as needed or requested to
assist Consumer/PCR and caregiver with program
compliance.
Agency is responsible to provide backup
caregivers.
ISO assists Consumer/PCR to create backup and
emergency plan w/choice of back up caregiver(s)
and plan for other emergencies. Consumer/PCR is
responsible for scheduling back up caregiver(s).
• The Agency maintains control of how the
Consumer/PCR directs how the services are
provided.
• Available where Provider Agencies are
located or provide services.
• Available Statewide
Agency must refer to ISO or Home Health
Agency for skilled services.
ISO may provide Self-Directed Skilled services
authorized under NRS 629.091.
